How to register with us

Register online

Use this service to register with a GP surgery for the first time or to change your current GP surgery. This link will open in a new window.

How long it takes

It takes around 15 minutes to complete a registration. You need to complete the process in one session. If you are registering more than one person make sure to give yourself enough time.

Your chosen GP surgery should register you within 5 days of receiving your details.

If you are registering more than one person

For multiple or family registrations you need to complete a registration for each person before submitting them all at once.

If you are registering children under the age of 16 years it is important that you register yourself first.

Before you start

You do not need your NHS number, but it could make registration easier.

We may ask you for:

  • details of previous GP surgery
  • basic health and medical information

The information you provide will be used to identify the correct medical records. It will also help the GP surgery to offer relevant and appropriate healthcare services.

Long Term Conditions

If you have certain long-term conditions (conditions which impact over a long period of time such as diabetes and heart diseases), you will be offered and encouraged to attend a Yearly Health Check to better care for your health. We will work with you to improve your health by considering what matters to you. We will consider your individual needs and preferences as well as best practice for your long-term condition. Our goals are that patients with long term conditions manage their health with confidence, reduce the risk of being admitted to hospital, and have a better quality of life.

If this is for you, we will contact you via text, phone call, or letter to organise your appointments. We will contact patients over the year so please do not worry if you do not hear from us right away.

Who is this for?

Initially, we will cover nine long-term conditions:

  • Cardiovascular disease (for example, strokes, health failure, ischaemic heart disease, and peripheral artery disease)
  • Diabetes
  • Hypertension (High blood pressure)
  • Hyperlipidaemia (High cholesterol)
  • Non-alcoholic fatty liver disease
  • Atrial fibrillation
  • Chronic kidney disease
  • Asthma
  • Chronic obstructive pulmonary disease.

In the future, we will look to expand this to cover more long-term conditions.

Why Vaccines are Important

Vaccination is the most important thing we can do to protect ourselves and our children against ill health. They prevent millions of deaths worldwide every year. Since vaccines were introduced in the UK, diseases like smallpox, polio and tetanus that used to kill or disable millions of people are either gone or are now very rarely seen.  Other diseases like measles and diphtheria have reduced to a very low number of cases each year since vaccines were introduced. These cases are often related to travel. However, if people stop having vaccines, it’s possible for infectious diseases to quickly spread again.

Polio

You may have followed the news that polio has been found in sewage in England and a nationwide Polio Vaccination Campaign is being started in its wake. If your child is not up-to-date with their scheduled immunisation we recommend that you get in touch so we can arrange an appointment for them.

Adult Vaccinations and COVID

Pneumococcal and Shingles

We provide patients with pneumococcal and shingles vaccinations. For more information on availability, please see the following NHS websites or ask staff.

COVID Vaccination

COVID-19 Vaccination Uptake (fully or partially vaccinated):

  • Age over 80: 95.1%
  • Age 75-79: 91.4%
  • Age 70-74 and high risk patients: 86.3%
  • Age 65-69: 87.1%
  • Age 16-64: 74%
  • Learning Disability patients: 80% fully or partially vaccinated
  • BAME Patients: 61% fully or partially vaccinated
  • Housebound patients: 94%

Influenza Vaccination

Influenza Vaccination Uptake:

  • Age 65 and over: 76.1%
  • Age 16-64: 32.8%

North Central London CCG has taken a proactive approach to COVID vaccination at all risk and non-risk groups. The practice is currently participating in the second round of housebound vaccination arrangements. If you have recently become housebound, please let us know so we can forward your details to the relevant team.

Private Fees

Our private fees follow BMA guidance and are available on request. Please note that we all requests are dealt with in a timely manner, within 1 calendar month from the date of request. This is to enable us to manage our workload as we are a busy GP practice.
